The upright position, such as kneeling or sitting, is effective in decreasing back pain during labor because it helps to utilize gravity to facilitate the descent of the baby through the birth canal. This position allows the pelvis to open wider, enhancing the alignment of the spine and potentially alleviating pressure on the lower back. Additionally, being upright can enhance uterine contractions' effectiveness and encourage the labor process to move forward while providing more comfort and control for the laboring mother.

In contrast, the supine position often leads to increased pressure on the spine and can create discomfort during labor. Lying flat on the back can compress major blood vessels, leading to decreased circulation and increased pain. A reclining position may not provide the same benefits as an upright position for back pain relief due to similar pressures placed on the spine. Therefore, the upright position is considered the best choice for managing pain in the back during labor.
